Family and Medical Leave

Employees often will need time away from work for many different types of personal situations. Whether it’s time off from work to take care for a new child, or deal with a serious health situation involving a family member or themselves. Employees will need to request time off from their supervisor and by completing the Leave Request.  Federal and State protected leave laws ensure that employees are granted time off for leave that is needed for qualifying reasons. 

Employees are required to notify Human Resources at least thirty (30) days prior to the needed leave. If leave was not anticipated notice must be provided within 48 hours of the leave. 

Below are some protected leave types.  Each one requires the Employee Leave Request to be completed and return to Human Resources for processing. Below you will find additional document which are also required to complete your leave request.  

  • Federal Family Medical Leave (FMLA) and Oregon Family Leave Act (OFLA)
  • Paid Family and Medical Leave - Paid Leave Oregon (PLO)  
  • Pregnancy Disability Leave (OFLA/PLO but not currently)
  • Parental Leave (Bonding Leave) (FMLA/PLO)
  • Bereavement Leave (OFLA)
  • Military Leave (FMLA)
